Club Forms

Please download the required form to your computer, complete it as required and then give/send it to the person named.

• Club Membership form- please hand completed form to the Secretary, Tracey McDonald, who will usually be in the cafe at the Aubigny Centre on Wednesday evenings at 6.45pm.
• SASA Membership form- please hand completed form to the Secretary, Tracey McDonald, who will usually be in the cafe at the Aubigny Centre on Wednesday evenings  at 6.45pm. (Note: no money needs to be sent with this form, the fee is included in your monthly pool fee.)
• Standing Order mandate - complete and send to your bank or set up online.
